# Questions for the customer (SAMPLE, based on fictional "Ember & Fern Co." data)

These are the open items from component-mapping.csv (rows with needs_customer_check = yes). A real brand would answer them before the totals are used in a report. We prepare data; the customer decides what to report.

## Scope (the customer decides; we do not)
1. Which of your products and packaging are covered for Oregon reporting, and is your company obligated or exempt? We do not decide this. Please confirm with DEQ / the program administrator or your own adviser.
2. Do shipping materials (corrugated shipper boxes, mailers, fill, tape) count as covered packaging for you? This sample includes them; remove them if not.
3. Is the unit count "units shipped into Oregon in the prior calendar year" from one consistent source (orders by ship-to state)? Returns, gifts, wholesale and marketplace sales included or not?

## Materials we could not determine from the description
4. Metal "tin" (tin lid, candle tin, salve tin, coffee tin): steel or aluminum? Any plastic liner or coating? Dimensions (small-format check)?
5. Plastic resin for: poly bag, poly mailer, squeeze tube, flip cap, screw lid, plastic snap lid, shrink band, dropper collar, pump parts. Please give the resin code or supplier spec sheet.
6. "White foam cushion block": EPS (polystyrene), PE foam or other?
7. "Compostable mailer": what is it made of (PLA, PBAT blend, paper)? Please send the supplier's spec.
8. Multi-material items: foil-lined coffee pouch, squeeze tube (may be laminate), dropper assembly, pump dispenser. Spec sheet with layers or component breakdown by material and weight?
9. Coatings: are the paperboard cartons, sleeves, mailers and crinkle fill uncoated? Any plastic windows, wax, or poly lamination?
10. "Kraft mailer box": corrugated or solid paperboard?
11. Molded pulp insert and bamboo lid: confirm material; any plastic lining or silicone seal?

## Weights
12. Were component weights measured on a scale (grams, packaging only, empty) or taken from supplier specs? Which components vary between production batches?
13. Items we left out: paper labels, hang tags, adhesive, inks, product inserts/cards. Do you want them included?

## Format
14. Do you want totals in the 60-category list (Oregon, 2025/2026 fee schedule) only? The category list changes for 2027 fees (two categories split).
